When a clinician sits down with a client for a psychological assessment, one of the most consequential decisions they make isn’t what to ask – it’s how to ask it. The format of the interview shapes everything: the data collected, the diagnostic accuracy, the depth of understanding, and even the therapeutic relationship itself. Clinical interviews exist on a spectrum from tightly controlled to completely open-ended, and knowing where on that spectrum to operate can determine the outcome of an entire assessment.

Why interview format matters

The clinical interview is the most widely used assessment tool in mental health. Whether it is gathering background history, diagnosing a disorder, or understanding a client’s daily functioning, the interview provides information that no questionnaire or standardized test can fully replicate. But interviews are not one-size-fits-all. Clinical interviews vary considerably in how much structure they carry, ranging from a rigid, fully scripted format to a free-flowing conversation. Three main formats define this spectrum: structured, semi-structured, and unstructured interviews. Each has a distinct logic, specific tools associated with it, and a particular set of trade-offs that clinicians must weigh.

Structured interviews: precision and consistency

A structured interview follows a fixed set of pre-established questions, asked in the same order, using the same wording, for every client. The interviewer does not deviate from the schedule except to clarify a question’s meaning, and probing beyond the answers received is not part of the format. The result is a highly standardized process that generates comparable, quantifiable data across different clients, clinicians, and settings.

The primary strength of this format is reliability. Because every respondent is asked identical questions in the same sequence, the risk of interviewer bias is substantially reduced. This makes structured interviews especially well-suited for large-scale research, epidemiological studies, and diagnostic contexts where consistency is paramount. They are frequently applied when assessing predetermined constructs or diagnosing conditions, often using rating scales, checklists, or Likert-type response formats to quantify responses.

The most prominent example is the Structured Clinical Interview for DSM (SCID). The SCID is a semi-structured interview guide designed to yield diagnoses according to the DSM criteria, administered by a trained clinician or mental health professional familiar with DSM classification. The SCID was originally developed because existing diagnostic interviews did not conform to the operationalized DSM-III criteria, and because it was argued that valid diagnosis required a clinician capable of rephrasing questions, seeking clarification, and applying judgment. The current version, SCID-5, is broken down into separate modules corresponding to DSM-5 diagnostic categories, with entry questions that allow the interviewer to skip sections when criteria are clearly not met. Research on the SCID-5 Clinician Version has confirmed excellent reliability and high diagnostic specificity, supporting its use in both daily clinical practice and research settings.

The key limitation of structured interviews, however, is inflexibility. The answers they generate tend to lack depth because only closed questions are asked, which means the clinician won’t know why a person behaves a certain way. The format can feel impersonal, and it may miss important clinical nuances that only emerge in a more open exchange.

Semi-structured interviews: flexibility within a framework

Semi-structured interviews occupy the middle ground. They begin with a core set of pre-planned questions or topic areas, but the clinician retains the freedom to follow up on responses, probe further, adjust the sequence, and explore themes that arise organically. Semi-structured interviews are considered semi-structured precisely because they require professional judgment and discretion in their administration and scoring – they are not a mechanical recitation of questions.

The interviewer’s responsibility is not simply to record a client’s responses but to assess whether a particular symptom or trait is actually present. This demands clinical expertise, sensitivity to the client’s mood and defensiveness, and the ability to ask follow-up questions that go beyond the script. Semi-structured interviews are most useful when addressing “what,” “how,” and “why” research questions, and both qualitative and quantitative analyses can be performed on the data they produce.

A widely used example is the Hamilton Rating Scale for Depression (HRSD). The HRSD is a semi-structured interview containing 17 items that assess depressive symptoms experienced over the preceding week, and it is regarded as the gold standard for evaluating major depressive disorder in psychotherapy and psychiatric research. The scale requires a trained clinician to rate symptom severity based on both direct questioning and clinical observation – capturing verbal reports alongside behavioral cues like tone, posture, and affect. A structured interview guide for the HRSD (SIGH-D) was specifically developed to standardize administration and improve inter-rater reliability across items. Research has confirmed that structured versions of the HAM-D display the highest inter-rater and test-retest reliability compared to purely unstructured administration.

The semi-structured format is particularly valuable for diagnosing conditions that require nuanced understanding – depression, personality disorders, anxiety disorders – where both consistency and depth matter. Clients often feel more comfortable in semi-structured interviews because they are free to elaborate on their answers, which can improve rapport and encourage disclosure. The trade-off is that the format still carries some potential for interviewer bias and requires a higher level of clinical skill than a fully structured approach.

Unstructured interviews: depth and clinical judgment

Unstructured interviews have no fixed set of questions. The clinician creates an open, conversational environment where the client’s own responses guide the direction of the dialogue. Sometimes called “discovery interviews,” unstructured interviews resemble a guided conversation more than a formal procedure. The interviewer modifies questions to suit the client’s specific experiences and circumstances, making each session genuinely individualized.

This format is particularly valuable during initial intake sessions and in situations where the goal is to understand the full complexity of a person’s experience – their history, relationships, coping patterns, and subjective world – rather than to confirm or rule out a specific diagnosis. Unstructured clinical interviews typically cover a broad range of domains including identifying information, education and work history, social and relationship background, presenting problems, family history, mental and physical health history, substance use, and behavioral observations. Crucially, what the clinician observes – tone of voice, body language, affect, eye contact – contributes clinical data alongside what the client says.

The major strength of unstructured interviews is their depth and flexibility. They can uncover insights that standardized tools may entirely miss and allow the clinician to respond to the unique needs of the individual. However, this format comes with significant limitations. Unstructured clinical interviews rely entirely on the training, expertise, and conscientiousness of the interviewer for accuracy, and are notoriously unreliable, idiosyncratic, and prone to false assumptions and attributional errors in research settings. Without standardization, comparing findings across clients or studies becomes difficult, and the assessment is only as good as the clinician conducting it.

Comparing the three formats

Each format serves a different purpose, and the choice between them is not a matter of one being better than the others – it depends entirely on the clinical or research objective.

Structured interviews are best when consistency, comparability, and reliability are the top priorities – for instance, in large research studies, epidemiological surveys, or when a standardized diagnostic decision is needed quickly. Their limitations lie in the loss of depth and the inability to capture the complexity of individual experience.

Semi-structured interviews are the most commonly used format in clinical psychology research and practice. They offer a practical balance: enough standardization to ensure key domains are covered and data can be compared, enough flexibility for the clinician to follow meaningful leads and probe nuanced responses. They are the format of choice when diagnosing complex conditions that require both structured criteria and individualized exploration.

Unstructured interviews are most appropriate when the priority is understanding the person holistically – their history, subjective experience, and the unique context of their presenting concerns. They are almost universal during initial intake assessments but require a skilled, experienced clinician to generate reliable and clinically useful information.

Choosing the right format in practice

In real-world clinical settings, these formats are rarely used in complete isolation. A clinician might begin with an unstructured intake to build rapport and gather broad history, then deploy a semi-structured tool like the HRSD to assess symptom severity, and later use a structured instrument like the SCID for formal diagnosis. The skill lies in knowing what each format offers and matching it to the demands of the moment.

Context also matters significantly. A busy outpatient clinic with time constraints may favor structured or semi-structured tools for efficiency. A trauma-focused practice may prioritize unstructured approaches to allow clients to tell their stories at their own pace. Research settings almost always require at least semi-structured formats to ensure scientific comparability. Compared to paper-and-pencil questionnaires, semi-structured interviews allow for the probing and clarification of responses, and are less affected by respondents’ reading level – an important consideration in diverse clinical populations.

Ultimately, the format of a psychological assessment interview is not a bureaucratic detail – it is a clinical decision with direct consequences for diagnostic accuracy, therapeutic alliance, and the quality of care a client receives. Understanding the strengths and limitations of each format is foundational knowledge for any practitioner working in mental health assessment.
