When a person walks into a psychiatrist’s office with racing thoughts, persistent sadness, or experiences that seem completely disconnected from the world around them, the clinician’s first task is to determine what is happening – and how to categorize it. Unlike most medical conditions, mental disorders rarely come with a blood test or a scan that gives a clear diagnosis. Instead, clinicians rely on a structured set of principles to classify what they observe. These principles – rooted in clinical presentation, the presence or absence of brain pathology, and the severity of the break with reality – form the backbone of how mental disorders are systematically understood and diagnosed.

Why classification matters in mental health
Classification is not just an academic exercise. Without shared categories, two psychiatrists in different countries could apply entirely different diagnoses to the same patient. Research published in World Psychiatry notes that changes to modern classification systems like the DSM-5 were driven by advances in neuroscience, clinical need, and the goal of ensuring that clinicians across the globe communicate in the same diagnostic language. The two most widely used systems today – the Diagnostic and Statistical Manual of Mental Disorders (DSM-5-TR) from the American Psychiatric Association and the International Classification of Diseases (ICD-11) from the World Health Organization – both depend on consistent classification principles to make diagnosis reliable and treatment decisions meaningful.
But before these modern systems existed, psychiatry developed its frameworks organically, through clinical observation and debate. Understanding the foundational principles – clinical presentation and course, the organic-functional distinction, and the psychosis-neurosis divide – helps explain why psychiatry classifies disorders the way it does today.
Clinical presentation and course: the starting point of classification
Because mental disorders rarely have clear biological markers that can be confirmed through a laboratory test, the primary basis for classification is what clinicians can directly observe and what patients report. This is called symptom-based or descriptive classification.
The DSM is designed to focus primarily on the signs and symptoms of mental disorders rather than their underlying causes, since the pathophysiological processes behind most psychiatric conditions remain incompletely understood. This approach means that a diagnosis is made based on the pattern of symptoms – how many are present, how long they have persisted, and how much they impair the person’s ability to function in everyday life.
The role of illness course
Beyond symptoms at a single point in time, the course of illness – how a disorder begins, progresses, and resolves – is also central to classification. A landmark framework proposed by Robins and Guze in 1970 outlined key principles for establishing the validity of clinical syndromes, including distinctiveness from other disorders, a predictable clinical course, and patterns that run in families. This helped shift psychiatric classification away from vague descriptions toward more precise, operationally defined criteria.
For example, schizophrenia is classified in part because of its characteristic onset pattern (typically in late adolescence or early adulthood), its course (often chronic, with periods of relapse and partial remission), and its specific cluster of symptoms. A single episode of unusual behavior tells us less than a pattern of recurring symptoms over time. This is why duration is built into many diagnostic criteria – a psychotic episode lasting only a day is categorized differently from one that persists for months.
The symptom-and-course approach does have a limitation: it can result in what clinicians call comorbidity, where a single patient meets criteria for more than one disorder simultaneously, because symptoms overlap across categories. Researchers have identified reducing this artifactual comorbidity as one of the significant ongoing challenges for both the ICD and DSM classification systems.
Organic vs. functional disorders: is the brain the cause?
One of the oldest and most debated principles in psychiatric classification is the distinction between organic and functional disorders. At its core, this division asks a specific question: can we identify a physical abnormality in the brain that explains the psychological symptoms?
What organic disorders mean
According to a foundational definition in psychiatry, organic disorders with psychosis are caused by structural defects or physiologic dysfunction of the brain. These are conditions where a demonstrable physical cause can be identified – such as dementia due to Alzheimer’s disease, psychiatric symptoms following a stroke, or psychosis triggered by a brain tumor. In clinical settings, clinicians describe organic disorders as those with “an identifiable physical aetiology” – something visible on imaging, detectable in a blood test, or traceable to a clear physiological process in the brain.
A concrete example: a patient who develops paranoid delusions following a confirmed case of herpes simplex encephalitis has an organic psychosis. The psychiatric symptoms arise from a known neurological cause.
What functional disorders mean
Functional disorders, by contrast, imply no diagnosable pathophysiology contributing to the disorder’s cause. Schizophrenia, bipolar disorder, and major depressive disorder were historically classified as functional because no specific, causative brain lesion could be identified at the time these categories were established.
The term has a complex history. Historically, by around 1900, most psychiatrists believed that the insanities were of organic origin, while the neuroses were psychological in nature – a near reversal of the views held a century earlier. The concept of “functional disorder” was sometimes used to describe conditions where organic changes were suspected but undetectable with available technology.
Why this distinction is now considered outdated
Modern neuroscience has significantly complicated this neat divide. It is now generally accepted that the so-called functional psychoses, including schizophrenia, are themselves organic in the sense that they reflect an abnormality of brain function and are no longer regarded as purely psychologically determined. Brain imaging studies show structural and functional differences in conditions once labeled strictly “functional.”
As a result, the diagnostic category of “organic disorders” was officially removed from the psychiatric nosology in DSM-IV, published in 1994. Neither the DSM-5 nor ICD-11 formally uses the organic/functional label. Instead, conditions are now described as “secondary to” or “due to another medical condition,” though the practical reasoning behind the distinction remains essentially the same.
More recent diagnostic manuals have attempted to de-emphasize the functional-organic distinction, though researchers note that the changes are largely terminological – the underlying logic of asking whether a physical cause can be identified persists in day-to-day clinical practice.
Psychoses vs. neuroses: severity and contact with reality
Another foundational classification principle divides mental disorders based on how profoundly they disrupt a person’s relationship with reality. This gives us the traditional distinction between psychoses and neuroses.
Understanding psychosis
Psychosis is not a single disorder – it is a description of a severe mental state. Psychosis refers to symptoms that show a person has some kind of disconnection from reality. The hallmark features are hallucinations (perceiving things that are not there – hearing voices, seeing images), delusions (fixed, false beliefs that resist logical correction), and disorganized thinking. Disorders such as schizophrenia and certain presentations of bipolar disorder can involve psychotic episodes.
A particularly important feature of psychosis is the concept of anosognosia – the lack of insight into one’s own condition. This goes beyond denial; the person genuinely cannot recognize the symptoms of their illness. This is why psychotic individuals often resist treatment – they do not believe anything is wrong.
Understanding neurosis
Neurosis, by contrast, involves significant emotional distress but with preserved contact with reality. The DSM-II described neuroses as conditions where “anxiety is the chief characteristic”, and where neurotic symptoms manifest without gross distortion of external reality or gross personality disorganization. A person with severe generalized anxiety, obsessive-compulsive disorder, or a specific phobia experiences considerable suffering – but they know they are experiencing it. Their grip on shared reality remains intact.
In neurosis, the individual preserves their link with reality, maintains an organization of personality, maintains their grasp over social relations, and has preserved insight into their problems. They may know their fear of contamination is excessive, or that their constant worry is disproportionate – even if they cannot stop it.
The core contrast
The clearest way to understand the distinction is through the concept of insight. In neurosis, insight is preserved – the person recognizes their distress as coming from within themselves. In neurosis, the person usually recognizes that their anxiety or obsessions are irrational or excessive, even if they cannot control them. In psychosis, insight is typically lost – the delusions and hallucinations feel completely real, and the person cannot distinguish them from genuine experience.
In neurosis, suffering occurs within a real framework, while in psychosis the link between emotion and reality is broken. This difference directly shapes treatment: neurosis is most often addressed through psychotherapy, behavioral interventions, and in some cases medication. Psychosis typically requires antipsychotic medication alongside psychological support.
Are these categories still used today?
The term “neurosis” was officially dropped from the DSM when DSM-III was published in 1980, largely because the term had become too broad and theoretically loaded. Neurosis is not a term included in the DSM-5, and is not typically used in clinical diagnosis today. What were once called “neuroses” are now classified more precisely – as anxiety disorders, obsessive-compulsive disorder, somatic symptom disorders, and related conditions.
However, the conceptual distinction between disorders that do and do not involve a break with reality remains foundational to psychiatric thinking. The psychosis-neurosis divide still shapes how clinicians assess severity, choose treatments, and understand prognosis. The utility of these terms in psychiatry has maintained a dichotomous stance since the times of Freud and Jung, and while the labels have been refined, the clinical logic they encode has not disappeared.
From principles to modern practice
The three principles described here – classifying by clinical presentation and course, distinguishing organic from functional origins, and separating psychosis from neurosis – are not isolated concepts. They worked together historically to build the scaffolding of modern psychiatric classification. Today’s systems like the DSM-5-TR and ICD-11 have refined these principles, replacing broad categories with more precise, symptom-specific, and evidence-based criteria. The organic/functional binary has largely been replaced by a biopsychosocial model, and the neurosis category has been disaggregated into specific disorder clusters. But the underlying questions – What symptoms are present? How severe is the disruption to reality? Is there an identifiable physical cause? – remain at the heart of every psychiatric evaluation.
Classification systems serve a purpose far beyond academic neatness. Advances in understanding of the individual-level and societal-level causal mechanisms that contribute to vulnerability to mental disorder may ultimately lead to improved classification systems, and in turn to better individualized care and improved global mental health. Every refinement in how disorders are classified is ultimately a step toward better treatment for people who need it.
