The biopsychosocial model: a shared foundation
For most of medical history, the dominant framework was purely biomedical: illness had a physical cause, and treatment meant targeting that physical cause. This model worked well for infectious diseases and acute injuries, but struggled to explain conditions like chronic pain, unexplained fatigue, or the wide variation in how patients recovered from the same illness.
That gap gave rise to the biopsychosocial model, introduced by psychiatrist George Engel in 1977. Psychosomatic medicine is an interdisciplinary subspecialty that examines how emotional and mental processes affect physical health. It gained serious traction in the late twentieth century as modern medicine grew increasingly aware that stress, anxiety, and psychological disorders can substantially affect a person’s physical condition and their ability to recover from illness. Physical problems can also worsen mental health, creating a cycle that neither medicine nor psychology can fully address alone.
Common psychosomatic conditions
Research in psychosomatics has identified several conditions where psychological factors play a significant role in onset and progression. These include:
- Irritable Bowel Syndrome (IBS): Psychological factors such as stress, anxiety, and a history of trauma can trigger or worsen gastrointestinal symptoms. Treatments that incorporate psychological interventions, including gut-directed hypnotherapy and relaxation techniques, have shown meaningful results.
- Chronic pain conditions: Fibromyalgia, tension headaches, and lower back pain often have significant psychosomatic components. Some researchers suggest that lower back pain and hypertension may be directly related to everyday psychological stress.

As Psychology Today notes, the strongest perspective in psychosomatic medicine holds that attempting to categorize physical illnesses as either purely biological or purely psychosomatic is increasingly obsolete – almost all physical illnesses have mental factors that shape their onset, course, and response to treatment.
Psychoneuroimmunology: the science behind the connection
One of the most exciting developments in this space is psychoneuroimmunology (PNI) – a field that studies how the mind, nervous system, and immune system communicate with each other. PNI research shows that these systems are linked through ligands – hormones, neurotransmitters, and peptides – meaning that psychological states like stress or depression have measurable, biological effects on immune function. This science has provided the physiological evidence base that psychosomatic medicine needed to be taken seriously within mainstream medical practice.
The case for holistic patient care
The evidence makes a compelling case for treating patients as whole people, not collections of separate symptoms. A review published in PLOS Mental Health argues that addressing the global mental health crisis requires a paradigm shift – from models focused narrowly on disease toward holistic approaches that account for the biological, psychological, social, environmental, and even lifestyle dimensions of health. Poor sleep, inadequate nutrition, and chronic stress are identified as significant contributors to mental health conditions that are often overlooked when the focus is solely on pharmacological treatment.

Integrated medical-behavioral healthcare has also been shown to result in substantial reductions in overall medical costs, making the case for collaboration not only clinically sound but practically important for healthcare systems worldwide.
